Continuing Care e-Health (CCeH) supports a diverse group of clients including Community Care Access Centres, Home Care & Support Providers, Community Mental Health & Addiction organizations and Long-Term Care Homes in Ontario. Continuing Care referrals have been largely dependent on paper, fax and e-Forms. With so many types of care services available and with more than 3 million referrals and service orders processed annually, the existing labour intensive referral system had inefficiencies that caused frustration and service delays for care providers and clients.
The CCeH Solutions Delivery Group required an e-Referrals technology solution that could electronically transmit Continuing Care referrals for action, while tracking and collecting event information for service delivery wait time measurement. The solution required the use of HL7 messaging standards and needed to communicate with the existing messaging infrastructure. The system would be rolled out across the province to support referrals for the entire Continuing Care sector.
Intelliware was selected by CCeH to build a web-based application that could gather information about a care referral and format the referral data using HL7 standard messages. Intelliware was able to deliver the application in the time frame required because of existing Intelliware expertise with HL7 v2.x and v3, and existing Intelliware utilities and frameworks that could handle the construction and marshalling of HL7 messages. Additionally, Intelliware’s Agile development approach was a key factor in selection for this project and in the ability to deliver in the timeframes required. The e-Referrals application has been launched with Continuing Care clients.
